Provide the following in the notes at checkout:
Name
Age
Height
DOB
Phrase for top of sign (forty & fabulous for example)
8 things you love
# of teeth OR # of anything you want, for example 3 kids, 10 tattoos.
Favorite food or replace with Greatest Accomplishment
Font color - if you want different then the listing photo.
Stripes - please state if you want the stripe background vertical or horizontal. Listing photo is vertical
You will receive a proof for your review within 2-3 business days. Once you receive your proof, if you have any changes just let me know and the edit will be made.
